After printing the form please fill it out to the best of your knowledge. If you do not know what size to order, please call us and we will be more than happy to guide you in the right direction according to your body measurements. Mail this form to our store with your deposit, which should be a check or money order made out to Affordable Bridal, Inc. We DO NOT accept credit card payments over the phone. Please make sure to give us the bride's first and last name so we can add your payment to her file.

** Please note: If we receive your form with the ''size requested'' spot empty or blank, we will ask you to mail us a new one with the correct info filled in.
