Please call the store with your measurements to determine your size of the dress since sizes are cut according to each company’s size chart. Those above 5’9” in height may require extra length, which is at an additional cost. Plus sizes require an additional cost as well, depending on the manufacturer.

** Please mail this form in with a personal check or money order for payment. We do not accept credit cards over the phone or through the mail due to the possibility of identity theft. A deposit of 75% is required to place the order, and the other 25% is due upon pick up. It is recommended to pay in full if you live out of town and are having the dress picked up by someone else or shipped to you, but is not necessary. If you would like us to ship the dress to you, please add on an additional $37.10 to the cost of the dress.
